GAME PLAN ATHLETICS PARTICIPANT WAIVER

 

I, individually and as the parent(s) or guardian of the athlete named above, a minor, ask that she be admitted to participate in this event sponsored by Game Plan Athletics, and Breyona Colley. In consideration of such admission, I do hereby release, discharge, and hold harmless of and from all causes, liabilities, damages, claims, or demands whatsoever on account of an injury or accident involving the said minor arising out of the minor’s attendance at the sporting event or in the even competition and/or activities held in connection with the sports event.

 

Participant Requirements and the Assumption of the Risk and Waiver of Liability Relating to COVID-19:

 

COVID-19 has been declared a worldwide pandemic by the World Health Organization. It is very contagious and is believed to be spread mainly from person-to-person contact. Game Plan Athletics, LLC (“GPA”) has put in place preventative measures to reduce the spread of the COVID-19 virus. However, GPA cannot guarantee that you or your child(ren) will not become infected. Further, attending GPA could increase your risk and your child(ren) risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I and my child(ren) may be exposed to or infected by COVID-19 by attending GPA and that such exposure of infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at GPA may result from the actions, omissions, or negligence of myself and others, including, but not limited to GPA employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself including, but not limited to, personal injury, disability, illness, death, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experienced or incur in connection with my child(ren)’s attendance at GPA or participation in GPA programming (“Claims’). On my behalf, and on the behalf of my cold(ren), i hereby release, covenant not to sue, discharge, and hold harmless GPA, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes and Claims based on the actions, omissions, or negligence of GPA, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any GPA program.

 

Media Policy

 

I grant permission to Game Plan Athletics, LLC, its agents, and its employees the irrevocable and unrestricted right to produce photographs and videos taken of my child, myself, and members of my family while participating with Game Plan Athletics for any lawful purpose including publication, promotion, illustration, advertising, trade, or historical archive in any manner or in any medium. I hereby release Game Plan Athletics and its legal representatives from liability for any violation or claims relating to said images or video. Furthermore, I grant permission to use statements of my child(ren), myself, or my family members given during an interview or evaluation with or without my name for the purpose of advertising and publicity without restriction to time limit or geographic area. I waive my right, child(ren)’s rights, and my family’s rights to any and all compensation stemming from the use of these materials.

 

Cancellation Policy

There are no refunds for canceled tryout registrations.
