Player Memberships -
all athletes are required to have current memberships for the 24-25 season. Families will receive a club invitation via email to purchase the OVR / USAV membership. Families are also required to purchase an AAU membership as well.

Lindsey’s Law - All athletes and families are required by Ohio law to watch the Lindsey’s law video yearly. Once viewed, please submit this form.

Ohio Concussion Awareness Law - All families are required to review the Ohio’s Return to play Law and the Ohio Department of Health (ODH) Youth Concussion information sheets. Once reviewed, please submit this form.
Required Parent/Player Paperwork
The following paperwork, required by all Athletes and Families, is due no later than December 1, 2025, for the 2025-2026 Season.

JVA Release of Liability - Families will need to complete player forms and can find a copy of this release HERE. On the left side of the page, choose the “CLICK HERE” option next to “To access your completed player forms” option. Once completed, the club will be sent a copy.

USAV Medical Release Form and Medical Waiver - All families are responsible for completing the medical release and medical waiver.
06.
OVR Junior Officiating Course - All athletes are required to complete all training OVR modules. All athletes need to be able to complete all jobs as these will be a rotating assignment to ensure that the same players are not always working during “work” matches.
